The information is requested for the purpose of matching children with specific interests they may have and to better help us communicate with and reach out to you and your family when we come in contact with organizations providing events and activities that are related to those interests.

  • Please submit a copy of the child's most recent report card or a letter from the child's teacher/counselor/principal that includes information regarding attendance and school performance. (must be on school letterhead.)

    Also, please include documentation of any community service completed by the child during the past 6 months.

    (If a scanner is not available you can submit a clear photo of the documents also.)
